This is to disable semaphore usage when on vGPU for now. Unfortunately
GVT-g hasn't fully enabled semaphore usage yet, so current guest with
semaphore use would cause vGPU failure.
Although current semaphore failure with vGPU can be simply resolved by
allowing cmd parser to accept MI_SEMAPHORE_WAIT command with address
audit, we're checking general usage of semaphore and how we should
handle it properly for virtualization in consider of function and
security concern. So we decide to request to disable it for now in
guest driver. Once GVT could support it, we would add new compat bit
to turn it on.

This moves ppgtt root hook out of scan and shadow function,
as it's only required at dispatch time. Also make sure this
checks against shadow mm to be ready, otherwise bail to fail
earlier.

As vGPU shadow ctx is loaded with guest context state, arbitrarily
submitting request in error workload dispatch path would cause trouble.
So don't try to submit in error path now like in previous code.
This is to fix VM failure when GPU hang happens.

When MI_FLUSH_DW post write hw status page in index mode, the index
value is in dword step and turned into address offset in cmd dword1.
As status page size is 4K, so can't exceed that.
This fixed upper bound check in cmd parser code which incorrectly
stopped VM for reason of invalid MI_FLUSH_DW write index.

This is to fix missed mmap range check on vGPU bar2 region
and only allow to map vGPU allocated GMADDR range, which means
user space should support sparse mmap to get proper offset for
mmap vGPU aperture. And this takes care of actual pgoff in mmap
request as original code always does from beginning of vGPU
aperture.

In commit 6bb2a2af8b ("drm/i915/gvt: Fix crash after request->hw_context change"),
forgot to handle workload scan path in ELSP handler case which was to
optimize scanning earlier instead of in gvt submission thread, so request
alloc and add was splitting then which is against right process.
This trys to do a partial revert of that commit which still has workload
request alloc helper and make sure shadow state population is handled after
request alloc for target state buffer.

This trys to make 'kvmgt' module as self loadable instead of loading
by i915/gvt device model. So hypervisor specific module could be
stand-alone, e.g only after loading hypervisor specific module, GVT
feature could be enabled via specific hypervisor interface, e.g VFIO/mdev.
So this trys to use hypervisor module register/unregister interface
for that. Hypervisor module needs to take care of module reference
itself when working for hypervisor interface, e.g for VFIO/mdev,
hypervisor module would reference counting mdev when open and release.
This makes 'kvmgt' module really split from GVT device model. User
needs to load 'kvmgt' to enable VFIO/mdev interface.

Handle guest mm access life cycle properly with mmget()/mmput().
As noted by Linus, use_mm() depends on valid live page table but
KVM's mmgrab() doesn't guarantee that. As vGPU usage depends on
guest VM life cycle, need to make sure to use mmget()/mmput() to
guarantee VM address access.

This trys to give new born vGPU with higher scheduling chance
not only with adding to sched list head and also have higher
priority for workload sched for 2 seconds after starting to
schedule it. In order for fast GPU execution during VM boot,
and ensure guest driver setup with required state given in time.
This fixes recent failure seen on one VM with multiple linux VMs
running on kernel with commit 2621cefaa42b3("drm/i915: Provide a timeout to i915_gem_wait_for_idle() on setup"),
which had shorter setup timeout that caused context state init failed.

Physical plane's tiling mode value is given directly as
drm_format_mod for plane query, which is not correct fourcc
code. Fix it by using correct intel tiling fourcc mod definition.
Current qemu seems also doesn't correctly utilize drm_format_mod
for plane object setting. Anyway this is required to fix the usage.

Our shadow context content is from guest but with masked control reg like
CTX_CONTEXT_CONTROL, we need to make sure all settings from guest would be set
when this context is on hw, this trys to force mask enable bits for all to
ensure every bits setting would be effective on hw.
One regression found related to once inhibit bit is set, gpu engine are working
on inhibit state until MI_LOAD_REG_IMM command or context image clear inhibit
bit with mask bit set to 1, and val bit set to 0. In gvt-g currently workload
has the highest priority, so gvt-g workload could trigger preempt context
easily, preempt context set inhibit bit, then gvt-g workload is scheduled in,
but gvt-g workload shadow context image usually doesn't set inhibit mask bit,
so gpu is still in inhibit state when gvt workload is running. This caused gpu
hang.

It's a bit confusing that page write protect handler is live in
mmio emulation handler. This moves it to stand alone gvt ops.
Also remove unnecessary check of write protected page access
in mmio read handler and cleanup handling of failsafe case.

We had previous hack that tried to accept either i915_reg_t or offset
value to access vGPU virtual/shadow regs which broke that purpose to
be type safe in context. This one trys to explicitly separate the usage
of typed mmio reg with real offset.
Old vgpu_vreg(offset) helper is used only for offset now with new
vgpu_vreg_t(reg) is used for i915_reg_t only. Convert left usage
of that to new helper.
Also fixed left KASAN warning issues caused by previous hack.

Need to check valid state for per_ctx bb and bypass batch buffer
combine for scan if necessary. Otherwise adding invalid MI batch
buffer start cmd for per_ctx bb will cause scan failure, which is
taken as -EFAULT now so vGPU would be put in failsafe. This trys
to fix that by checking per_ctx bb valid state. Also remove old
invalid WARNING that indirect ctx bb shouldn't depend on valid
per_ctx bb.

This exposes vGPU context hw id in mdev sysfs which is used to
do vGPU based profiling. Retrieved vGPU context hw id can be set
through i915 perf ioctl to set profiling for target vGPU.

From commit 73dec95e6b ("drm/i915: Emit to ringbuffer directly"),
copy_gma_to_hva() now returns copied data length instead of 0, so
need to change error return check for that.
Note: Looks this is caused by backmerge conflict resolving, so
4.11-rc4 is not impacted as commit 73dec95e6b ("drm/i915: Emit to
ringbuffer directly") is not in 4.11. But need to fix this before I
can apply 4.12 stuff against drm-intel-next correctly.

From commit a6508ded2a ("drm/i915: Use page coloring to provide the guard
page at the end of the GTT"), we no longer explicitly subtract guard page
at end for GGTT address space init, so shouldn't subtract that for vGPU
balloon too, as that will leave that end page to be available for
vGPU. Change balloon to cover full range too.
This fixes to use recent drm-intel tip kernel for guest OS. Found by GVT-g
cmd parser that guest kernel uses end page as scratch then try to run
MI_STORE_REG_MEM onto it.

This assigns resolution definition for each vGPU type. For smaller
resource type we should limit max resolution, so e.g limit to 1024x768
for 64M type, others are still default to 1920x1200.

Previous vGPU type create tried to determine vGPU type name e.g _1, _2
based on the number of mdev devices can be created, but different type
might have very different resource size depending on physical device.
We need to split type name vs. actual mdev resource and create fixed
vGPU type with determined size for consistence.
With this we'd like to fix vGPU types for _1, _2, _4 and _8 now, each
type has fixed defined resource size. Available mdev instances that could
be created is determined by physical resource, and user should query
for that before creating.

Previously intel_gvt_init() was called very early even before
MMIO initialization which had several drawbacks:
- Have to handle MMIO access for initial MMIO state dump if golden
state firmware is not available
- Hypervisor detection should depend on pvinfo only instead of detecting
hypervisor status.
- Don't know hw resource size e.g aperture, ggtt size to determine
for vGPU type, etc.
This trys to move intel_gvt_init() call late after required info
has already been initialized for GVT host.

Previous high mem size initialized for vGPU type was too small which caused
failure for some VMs. This trys to take minimal value of 384MB for each VM and
enlarge default high mem size to make guest driver happy.

Instead of partially depending on vfio pin/unpin pages interface if
mdev is available, which would result in failure if vfio is not
on. But replace with a wrapper which need to be fixed till mdev
support got fully merged.

From commit e95433c73a, workload status setting
was changed to only capture on error path, but we need to set it properly in
normal path too, otherwise we'll fail to complete workload which could lead
guest VM vGPU reset.

Switch to use new for_each_engine() helper to properly access
enabled intel_engine_cs as i915 core has changed that to be
dynamic managed. At GVT-g init time would still depend on ring
mask to determine engine list as it's earlier.

i915 core should only call functions and structures exposed through
intel_gvt.h. Remove internal gvt.h and i915_pvinfo.h.
Change for internal intel_gvt structure as private handler which
not requires to expose gvt internal structure for i915 core.

From Ben's AGP dependence removal change, "needs_dmar" flag has not
been properly setup for new chips using new GTT init function. This
one adds missed setting of that flag to make sure we do pci mappings
with IOMMU enabled.

It's cleaned before saving and re-initialized after restoring.
So don't need to save/restore it. And also new chip has new address
for hardware status page register, don't write to old address.

This restores cache behavior for default AGP_USER_MEMORY as
uncached, and leave default AGP_USER_CACHED_MEMORY as LLC only.
I've seen different cache behavior on one sandybridge desktop CPU vs.
another mobile CPU. Until we figure out how to detect the real cache
config, restore back to the original behavior now.
