Paolo Bonzini 13fb59276b kvm: x86: disable shattered huge page recovery for PREEMPT_RT.
If a huge page is recovered (and becomes no executable) while another
thread is executing it, the resulting contention on mmu_lock can cause
latency spikes.  Disabling recovery for PREEMPT_RT kernels fixes this
issue.

Commit a97b0e773e (kvm: call kvm_arch_destroy_vm if vm creation fails)
sets users_count to 1 before kvm_arch_init_vm(), however, if kvm_arch_init_vm()
fails, we need to decrease this count.  By moving it earlier, we can push
the decrease to out_err_no_arch_destroy_vm without introducing yet another
error label.

Commit 9121923c45 ("kvm: Allocate memslots and buses before calling kvm_arch_init_vm")
moves memslots and buses allocations around, however, if kvm->srcu/irq_srcu fails
initialization, NULL will be returned instead of error code, NULL will not be intercepted
in kvm_dev_ioctl_create_vm() and be dereferenced by kvm_coalesced_mmio_init(), this patch
fixes it.

Moving the initialization is required anyway to avoid an incorrect synchronize_srcu that
was also reported by syzkaller:

Paolo Bonzini b8e8c8303f kvm: mmu: ITLB_MULTIHIT mitigation
With some Intel processors, putting the same virtual address in the TLB
as both a 4 KiB and 2 MiB page can confuse the instruction fetch unit
and cause the processor to issue a machine check resulting in a CPU lockup.

Unfortunately when EPT page tables use huge pages, it is possible for a
malicious guest to cause this situation.

Add a knob to mark huge pages as non-executable. When the nx_huge_pages
parameter is enabled (and we are using EPT), all huge pages are marked as
NX. If the guest attempts to execute in one of those pages, the page is
broken down into 4K pages, which are then marked executable.

This is not an issue for shadow paging (except nested EPT), because then
the host is in control of TLB flushes and the problematic situation cannot
happen.  With nested EPT, again the nested guest can cause problems shadow
and direct EPT is treated in the same way.

Paolo Bonzini 9167ab7993 KVM: vmx, svm: always run with EFER.NXE=1 when shadow paging is active
VMX already does so if the host has SMEP, in order to support the combination of
CR0.WP=1 and CR4.SMEP=1.  However, it is perfectly safe to always do so, and in
fact VMX already ends up running with EFER.NXE=1 on old processors that lack the
"load EFER" controls, because it may help avoiding a slow MSR write.  Removing
all the conditionals simplifies the code.

SVM does not have similar code, but it should since recent AMD processors do
support SMEP.  So this patch also makes the code for the two vendors more similar
while fixing NPT=0, CR0.WP=1 and CR4.SMEP=1 on AMD processors.

Paolo Bonzini cf05a67b68 KVM: x86: omit "impossible" pmu MSRs from MSR list
INTEL_PMC_MAX_GENERIC is currently 32, which exceeds the 18
contiguous MSR indices reserved by Intel for event selectors.
Since some machines actually have MSRs past the reserved range,
filtering them against x86_pmu.num_counters_gp may have false
positives.  Cut the list to 18 entries to avoid this.

Paolo Bonzini 24c29b7ac0 KVM: x86: omit absent pmu MSRs from MSR list
INTEL_PMC_MAX_GENERIC is currently 32, which exceeds the 18 contiguous

Paolo Bonzini 833b45de69 kvm: x86, powerpc: do not allow clearing largepages debugfs entry
The largepages debugfs entry is incremented/decremented as shadow
pages are created or destroyed.  Clearing it will result in an
underflow, which is harmless to KVM but ugly (and could be
misinterpreted by tools that use debugfs information), so make
this particular statistic read-only.

Paolo Bonzini 1f4e5fc83a KVM: x86: fix nested guest live migration with PML
Shadow paging is fundamentally incompatible with the page-modification
log, because the GPAs in the log come from the wrong memory map.
In particular, for the EPT page-modification log, the GPAs in the log come
from L2 rather than L1.  (If there was a non-EPT page-modification log,
we couldn't use it for shadow paging because it would log GVAs rather
than GPAs).

Therefore, we need to rely on write protection to record dirty pages.
This has the side effect of bypassing PML, since writes now result in an
EPT violation vmexit.

This is relatively easy to add to KVM, because pretty much the only place
that needs changing is spte_clear_dirty.  The first access to the page
already goes through the page fault path and records the correct GPA;
it's only subsequent accesses that are wrong.  Therefore, we can equip
set_spte (where the first access happens) to record that the SPTE will
have to be write protected, and then spte_clear_dirty will use this
information to do the right thing.

Paolo Bonzini 6eeb4ef049 KVM: x86: assign two bits to track SPTE kinds
Currently, we are overloading SPTE_SPECIAL_MASK to mean both
"A/D bits unavailable" and MMIO, where the difference between the
two is determined by mio_mask and mmio_value.

However, the next patch will need two bits to distinguish
availability of A/D bits from write protection.  So, while at
it give MMIO its own bit pattern, and move the two bits from
bit 62 to bits 52..53 since Intel is allocating EPT page table
bits from the top.

Paolo Bonzini fd3edd4a90 KVM: nVMX: cleanup and fix host 64-bit mode checks
KVM was incorrectly checking vmcs12->host_ia32_efer even if the "load
IA32_EFER" exit control was reset.  Also, some checks were not using
the new CC macro for tracing.

Cleanup everything so that the vCPU's 64-bit mode is determined
directly from EFER_LMA and the VMCS checks are based on that, which
matches section 26.2.4 of the SDM.

Paolo Bonzini 50896de4be KVM: x86: always expose VIRT_SSBD to guests
Even though it is preferrable to use SPEC_CTRL (represented by
X86_FEATURE_AMD_SSBD) instead of VIRT_SPEC, VIRT_SPEC is always
supported anyway because otherwise it would be impossible to
migrate from old to new CPUs.  Make this apparent in the
result of KVM_GET_SUPPORTED_CPUID as well.

However, we need to hide the bit on Intel processors, so move
the setting to svm_set_supported_cpuid.

Paolo Bonzini 4c6903a0f9 KVM: x86: fix reporting of AMD speculation bug CPUID leaf
The AMD_* bits have to be set from the vendor-independent
feature and bug flags, because KVM_GET_SUPPORTED_CPUID does not care
about the vendor and they should be set on Intel processors as well.
On top of this, SSBD, STIBP and AMD_SSB_NO bit were not set, and
VIRT_SSBD does not have to be added manually because it is a
cpufeature that comes directly from the host's CPUID bit.

Paolo Bonzini 54577e5018 selftests: kvm: fix state save/load on processors without XSAVE
state_test and smm_test are failing on older processors that do not
have xcr0.  This is because on those processor KVM does provide
support for KVM_GET/SET_XSAVE (to avoid having to rely on the older
KVM_GET/SET_FPU) but not for KVM_GET/SET_XCRS.

Paolo Bonzini ed4e7b057e MAINTAINERS: add KVM x86 reviewers
This is probably overdue---KVM x86 has quite a few contributors that
usually review each other's patches, which is really helpful to me.
Formalize this by listing them as reviewers.  I am including people

In other news, Radim is going to leave Red Hat soon.  However, he has
not been very much involved in upstream KVM development for some time,
and in the immediate future he is still going to help maintain kvm/queue
while I am on vacation.  Since not much is going to change, I will let
him decide whether he wants to keep the maintainer role after he leaves.

Paolo Bonzini 9e5470fe2d scsi: virtio_scsi: implement request batching
Adding the command and kicking the virtqueue so far was done one after
another.  Make the kick optional, so that we can take into account
SCMD_LAST.  We also need a commit_rqs callback to kick the device if

Paolo Bonzini 8930a6c207 scsi: core: add support for request batching
This allows a list of requests to be issued, with the LLD only writing the
hardware doorbell when necessary, after the last request was prepared.
This is more efficient if we have lists of requests to issue, particularly
on virtualized hardware, where writing the doorbell is more expensive than
on real hardware.

The use case for this is plugged IO, where blk-mq flushes a batch of
requests all at once.

The API is the same as for blk-mq, just with blk-mq concepts tweaked to
fit the SCSI subsystem API: the "last" flag in blk_mq_queue_data becomes a
flag in scsi_cmnd, while the queue_num in the commit_rqs callback is
extracted from the hctx and passed as a parameter.

The only complication is that blk-mq uses different plugging heuristics
depending on whether commit_rqs is present or not.  So we have two
different sets of blk_mq_ops and pick one depending on whether the
scsi_host template uses commit_rqs or not.

Paolo Bonzini 88dddc11a8 KVM: nVMX: do not use dangling shadow VMCS after guest reset
If a KVM guest is reset while running a nested guest, free_nested will
disable the shadow VMCS execution control in the vmcs01.  However,
on the next KVM_RUN vmx_vcpu_run would nevertheless try to sync
the VMCS12 to the shadow VMCS which has since been freed.

This causes a vmptrld of a NULL pointer on my machime, but Jan reports
the host to hang altogether.  Let's see how much this trivial patch fixes.

Paolo Bonzini 9fd5887726 KVM: nVMX: reorganize initial steps of vmx_set_nested_state
Commit 332d079735 ("KVM: nVMX: KVM_SET_NESTED_STATE - Tear down old EVMCS
state before setting new state", 2019-05-02) broke evmcs_test because the
eVMCS setup must be performed even if there is no VMXON region defined,
as long as the eVMCS bit is set in the assist page.

While the simplest possible fix would be to add a check on
kvm_state->flags & KVM_STATE_NESTED_EVMCS in the initial "if" that
covers kvm_state->hdr.vmx.vmxon_pa == -1ull, that is quite ugly.

Instead, this patch moves checks earlier in the function and
conditionalizes them on kvm_state->hdr.vmx.vmxon_pa, so that
vmx_set_nested_state always goes through vmx_leave_nested
and nested_enable_evmcs.

vmx->loaded_cpu_state can only be NULL or equal to vmx->loaded_vmcs,
so change it to a bool.  Because the direction of the bool is
now the opposite of vmx->guest_msrs_dirty, change the direction of
vmx->guest_msrs_dirty so that they match.

Finally, do not imply that MSRs have to be reloaded when
vmx->guest_state_loaded is false; instead, set vmx->guest_msrs_ready
to false explicitly in vmx_prepare_switch_to_host.

Paolo Bonzini 1dfdb45ec5 KVM: x86: clean up conditions for asynchronous page fault handling
Even when asynchronous page fault is disabled, KVM does not want to pause
the host if a guest triggers a page fault; instead it will put it into
an artificial HLT state that allows running other host processes while
allowing interrupt delivery into the guest.

However, the way this feature is triggered is a bit confusing.
First, it is not used for page faults while a nested guest is
running: but this is not an issue since the artificial halt
is completely invisible to the guest, either L1 or L2.  Second,
it is used even if kvm_halt_in_guest() returns true; in this case,
the guest probably should not pay the additional latency cost of the
artificial halt, and thus we should handle the page fault in a
completely synchronous way.

By introducing a new function kvm_can_deliver_async_pf, this patch
commonizes the code that chooses whether to deliver an async page fault
(kvm_arch_async_page_not_present) and the code that chooses whether a
page fault should be handled synchronously (kvm_can_do_async_pf).

beyond the width of the fixed-function counter are reserved and must be

The guest_code of sync_regs_test is assuming that the compiler will not
touch %r11 outside the asm that increments it, which is a bit brittle.
Instead, we can increment a variable and use a dummy asm to ensure the
increment is not optimized away.  However, we also need to use a
callee-save register or the compiler will insert a save/restore around
the vmexit, breaking the whole idea behind the test.

(Yes, "if it ain't broken...", but I would like the test to be clean
before it is copied into the upcoming s390 selftests).

Paolo Bonzini 889e31e73a vhost-scsi: remove incorrect memory barrier
At this point, vs_tpg is not public at all; tv_tpg_vhost_count
is accessed under tpg->tv_tpg_mutex; tpg->vhost_scsi is
accessed under vhost_scsi_mutex.  Therefor there are no atomic
operations involved at all here, just remove the barrier.

Paolo Bonzini 65c4189de8 KVM: fix KVM_CLEAR_DIRTY_LOG for memory slots of unaligned size
If a memory slot's size is not a multiple of 64 pages (256K), then
the KVM_CLEAR_DIRTY_LOG API is unusable: clearing the final 64 pages
either requires the requested page range to go beyond memslot->npages,
or requires log->num_pages to be unaligned, and kvm_clear_dirty_log_protect
requires log->num_pages to be both in range and aligned.

To allow this case, allow log->num_pages not to be a multiple of 64 if
it ends exactly on the last page of the slot.

Paolo Bonzini 147b502bda wlcore: simplify/fix/optimize reg_ch_conf_pending operations
Bitmaps are defined on unsigned longs, so the usage of u32[2] in the
wlcore driver is incorrect.  As noted by Peter Zijlstra, casting arrays
to a bitmap is incorrect for big-endian architectures.

When looking at it I observed that:

- operations on reg_ch_conf_pending is always under the wl_lock mutex,
so set_bit is overkill

- the only case where reg_ch_conf_pending is accessed a u32 at a time is
unnecessary too.

This patch cleans up everything in this area, and changes tmp_ch_bitmap
to have the proper alignment.

Paolo Bonzini 98d9e858fa KVM: nVMX: Return -EINVAL when signaling failure in pre-VM-Entry helpers
Convert all top-level nested VM-Enter consistency check functions to
return 0/-EINVAL instead of failure codes, since now they can only
ever return one failure code.

This also does not give the false impression that failure information is
always consumed and/or relevant, e.g. vmx_set_nested_state() only
cares whether or not the checks were successful.

nested_check_host_control_regs() can also now be inlined into its caller,
nested_vmx_check_host_state, since the two have effectively become the
same function.

Based on a patch by Sean Christopherson.

Paolo Bonzini 9d609649bb KVM: vmx: print more APICv fields in dump_vmcs
The SVI, RVI, virtual-APIC page address and APIC-access page address fields
were left out of dump_vmcs.  Add them.

KERN_CONT technically isn't SMP safe, but it's okay to use it here since
the whole of dump_vmcs() is a single huge multi-line piece of output
that isn't SMP-safe.

Paolo Bonzini c2390f16fc selftests: kvm: fix for compilers that do not support -no-pie
-no-pie was added to GCC at the same time as their configuration option
--enable-default-pie.  Compilers that were built before do not have
-no-pie, but they also do not need it.  Detect the option at build
time.
