Dirty logging tracks sptes in 4k granularity, meaning that large sptes

This patch introduce lazy collapsing of small sptes into large sptes.
The rmap will be scanned in ioctl context when dirty logging is stopped,
dropping those sptes which can be collapsed into a single large-page spte.

I observed that DL tasks can't be migrated to other CPUs during CPU
hotplug, in addition, task may/may not be running again if CPU is
added back.
The root cause which I found is that DL tasks will be throtted and
removed from the DL rq after comsuming all their budget, which
leads to the situation that stop task can't pick them up from the
DL rq and migrate them to other CPUs during hotplug.
The method to reproduce:
schedtool -E -t 50000:100000 -e ./test
Actually './test' is just a simple for loop. Then observe which CPU the
test task is on and offline it:
echo 0 > /sys/devices/system/cpu/cpuN/online
This patch adds the DL task migration during CPU hotplug by finding a
most suitable later deadline rq after DL timer fires if current rq is
offline.
If it fails to find a suitable later deadline rq then it falls back to
any eligible online CPU in so that the deadline task will come back
to us, and the push/pull mechanism should then move it around properly.

sgs->group_capacity_factor is used to check if the group is overloaded.
When the child domain prefers tasks to go to siblings first, the
sgs->group_capacity_factor will be set lower than one in order to
move all the excess tasks away.
However, group overloaded status is not updated when

My system has 4 sockets and each socket has 15 cores and HT is

follows:

Socket#1 | 15-29, 75-89
Socket#2 | 30-44, 90-104
Socket#3 | 45-59, 105-119
Then llc_shared_mask of CPU#30 has 0x3fff80000001fffc0000000.
It means that last level cache of Socket#2 is shared with
CPU#30-44 and 90-104.
When hot-removing socket#2 and #3, each core of sockets is
numbered as follows:
| CPU#
Socket#0 | 0-14 , 60-74
Socket#1 | 15-29, 75-89
But llc_shared_mask is not cleared. So llc_shared_mask of CPU#30
remains having 0x3fff80000001fffc0000000.
After that, when hot-adding socket#2 and #3, each core of
sockets is numbered as follows:
| CPU#
Socket#0 | 0-14 , 60-74
Socket#1 | 15-29, 75-89
Socket#2 | 30-59
Socket#3 | 90-119
Then llc_shared_mask of CPU#30 becomes
0x3fff8000fffffffc0000000. It means that last level cache of
Socket#2 is shared with CPU#30-59 and 90-104. So the mask has
the wrong value.

To fix this, we cannot rely on the processor's virtual interrupt delivery,
because "acknowledge interrupt on exit" must only update the virtual
ISR/PPR/IRR registers (and SVI, which is just a cache of the virtual ISR)
but it should not deliver the interrupt through the IDT. Thus, KVM has
to deliver the interrupt "by hand", similar to the treatment of EOI in
commit fc57ac2c9c (KVM: lapic: sync highest ISR to hardware apic on
EOI, 2014-05-14).
The patch modifies kvm_cpu_get_interrupt to always acknowledge an
interrupt; there are only two callers, and the other is not affected
because it is never reached with kvm_apic_vid_enabled() == true. Then it
modifies apic_set_isr and apic_clear_irr to update SVI and RVI in addition
to the registers.

Enabling memory self refresh (SR) on 9xx needs to set additional
register bits. On 945, we need bit 31 of FW_BLC_SELF to enable the
write to self refresh bit and bit 16 to enable the write of self
refresh watermark. On 915, bit 12 of INSTPM is used to enable SR.
SR will take effect when CPU enters C3+ state and its entry/exit
should be automatically controlled by H/W, driver only needs to set
SR enable bits in wm update. But this isn't safe in my test on 945
because GPU is hung. So this patch explicitly enables SR when GPU
