Currently credit_refill and credit_max could be zero for a TC and that
is causing Tx hang for CEE mode configuration, so to fix that have at
min credit assigned to a TC and that is as what IEEE mode already does.

Because i40e_fcoe_ctxt_eof should never be called without
i40e_fcoe_eof_is_supported being called first, the EOF in fcoe_ctxt_eof
should always be valid and therefore we do not need to print an error
if it is not valid.
However, a WARN ON to easily catch any calls to i40e_fcoe_ctxt_eof that
aren't preceded with a call to i40e_fcoe_eof_is_supported is helpful.

Set different dev_port value 1 for FCoE netdev than the default zero
dev_port value for PF netdev, this helps biosdevname user tool to
differentiate them correctly while both attached to the same PCI
function.

Adds FCoE config option I40E_FCOE, so that FCoE can be enabled
as needed but otherwise have it disabled by default.
This also eliminate multiple FCoE config checks, instead now just
one config check for CONFIG_I40E_FCOE.
The I40E FCoE was added with 3.17 kernel and therefore this patch
shall be applied to stable 3.17 kernel also.

This patch adds FCoE ( Fibre Channel Over Ethernet ) code for
Intel XL710 adapters. This patch is limited to only new FCoE
offloads code in newly added files by this patch and then
following patches in the series modifies rest of the existing
driver to enable FCoE with i40e driver.

Currently FCoE is not supported with MFP modes, so this patch
makes sure it is disabled.
It is disabled by overriding HW FCoE capability, so that later
it can be re-enabled by just the SW upgrade.

Currently fc_fcp_timeout doesn't check FC_RP_FLAGS_REC_SUPPORTED
flag first, this prevents REC request ever going out at all
to the target having REC support. So this patches fixes the
fc_fcp_timeout by checking FC_RP_FLAGS_REC_SUPPORTED flag first.
The changed order won't cause any issue during clearing
FC_RP_FLAGS_REC_SUPPORTED on failed IO with target not supporting
FC_RP_FLAGS_REC_SUPPORTED, since retry on failed IO would succeed.

The FC-GS-3 sepc requires to wait for least 3 times R_A_TOV per
sec 4.6.1 "If the Requesting_CT does not receive a Response
CT_IU from the Responding_CT within three times R_A_TOV,
it shall consider this to be a protocol error."
This means added four new states with management server
could add significant delay with multiple retries
on default 12 second timeout(3 * R_A_TOV), so instead
just skip these states on very first timeout on any of
these states to not stuck with states for such longer
period.

Add exch timeout info to have debug log with exch timeout
value to match with retries, also add debug info
on exch timer cancel.
Added common fc_exch_timer_cancel() func and grouped this
along with fc_exch_timer_set() function, so that
added debug code is not repeated.

The libfc is used by fcoe but fcoe agnostic,
and therefore should not have any fcoe references.
So renaming fcoe_dev_stats from libfc as its for fc_stats.
After that libfc is fcoe string free except some strings for
Open-FCoE.org.

The libfc provides more flexibility and with that
we can monitor some more FC specific stats for
FC exches or FCP error cases, this patch add
such new FC stats.
The patch adds *only* FC specific new stats to
existing fc_host attribute container.
Added stats names are self explanatory as
existing FC stats already has, however anyway
still added commentary along their definition
to describe them.

Remove lport from net device and then do synchronize net device to flush
inflight rx frames for the lport before doing fcoe_percpu_clean.
In case of master lport, remove all rx packet handlers completely and
then only do fcoe_percpu_clean. This required splitting fcoe_interface_cleanup
to do remove part separately and for that added func fcoe_interface_remove
and then call it from fcoe_if_destory before doing fcoe_percpu_clean.
However if fcoe_interface_remove() is already called then
don't call again from fcoe_interface_cleanup() to preserve its
existing flows.
This patch along with Neil's other patch to avoid soft irq context
on ingress will avoid passing up frames on disabled lport as
discussed in this mail thread:-

A previous commit changed the mfs checking to ensure the new
mfs is less or equal to the mfs supported by the FCF. This
doesn't work for BRDCM cards as they set an mfs of 2048 regardless
of whether the switch returns a larger mfs.
This patch validates the new mfs against the upper and lower spec
defined boundries for a FCoE mfs.

Currently fc_host mfs is not getting updated in
case its changed during FLOGI and that leaves fc_host
to show its initial old value in sysfs, so instead
have fc_host mfs updated along with updating lport mfs
during FLOGI.
Also in case of bad mfs during flogi, error out
instead of continuing with flogi.
[ Changes made by Robert Love: condition to '>=' and
added printing of lport->mfs in DBG statement. FLOGI
resp processing failed without being able to compare
FCoE MFS 2112 against an incoming MFS of 2112 ]

Adds more cases to do flogi retry, now also retry
on getting bad response due to either no ELS response
or flogi response payload length not large enough.
In those cases flogi was not retried and that
was leaving lport offline.

Currently timer delay is large and is using msleep to avoid

Currently fcoe_ddp_min doesn't have default value
so by default not used, so setting up default value
as 4k as this works better by avoiding overhead
of programing DDP for small IOs.

Use real dev in case it has HW vlan acceleration
support since in this case the real dev would
do needed vlan processing, this way unnecessary
vlan layer processing avoided and it gives
slightly better IOPS with 512B size IOs.

Re-arrange its fields to avoid padding and have better
cacheline alignments.
Removed not used start_time, end_time and last_pkt_time
fields.
This all reduced this struct size to 448 from 480 and
that also reduced one cacheline on x86_64 beside
eliminating 8 pads. However kept logical fields together.

Current fc_eh_host_reset leaves lport offline
permanently due to FLOGI response getting
handled by LOGO response from last reset as both
had same exchange id.
So fix this by having end to end exches clean-up
using exchange abort along exches reset
done from fc_eh_host_reset. This would avoid
exchanges collision between the sessions across
the reset. In this case implicit login should have
done that but no aborting support for FIP
frames, so just wait till lport->r_a_tov before
restarting next flogi to ensure all exchanges
are good to use again for next session.

Cleanup to:
- have selection for all types of frames, not just FCP.
- remove redundant cpu_online check once fcoe_select_cpu called
as this is not required since later code flow check for offlined
cpu.
- Simplify fcoe_select_cpu() by removing unnecessary checks to
skip curr_cpu, this also fixes possibly infinite loop in case
of curr_cpu is the only cpu while iterating in the loop.
This cleanup mainly applies to target as incoming request are
mostly for target, therefore Kiran has verified the patch
with target also.

Drop the rx frame having xid with wrong cpu info
or received with xid not matching to our xid.
Not dropping such frame is causing panic as
that causes accessing data struct beyond their
bounds.

Post an FCH_EVT_LIPRESET event on lport reset as
as lport reset occurs on FIP cleat virtual link,
this could be due to change in fcoe vlan and this
event will allow user app fcoemon to switch to

Currently single PCI pool used across all CPUs and that
doesn't scales up as number of CPU increases, so this
patch adds per CPU PCI pool to setup udl and that aligns
well from FCoE stack as that already has per CPU exch locking.
Adds per CPU PCI alloc setup and free in
ixgbe_fcoe_ddp_pools_alloc and ixgbe_fcoe_ddp_pools_free,
use CPU specific pool during DDP setup.
Re-arranged ixgbe_fcoe struct to have fewer holes
along with adding pools ptr using pahole.

In this case fsp was freed before error handler was invoked,
this is fixed by having SRR fsp reference freed by exch
destructor so that fsp will be always held until it exch
is freed.
Also don't reset fsp->recov_seq since this is needed by
SRR error handler to do exch done.

In cases exch is already timed out then exch layer could
end up calling resp handler again for its response frame
received after timeout, though in this case fc_exch_timeout
handler would have already called resp with FC_EX_TIMEOUT.
This would cause REC response handler to release its
fsp pkt hold twice instead once and possibly similar issues
with other ELS exchanges in this race.
To avoid this race have resp updated under exch lock
in rx path, the resp would get set to NULL in case
of FC_EX_TIMEOUT under the same lock to prevent resp
callback after FC_EX_TIMEOUT.

Currently fcoe interface cleanup is done after ndo_fcoe_disable
and that prevents logoff going out to the peer, so this patch
moves all netdev cleanup and its releasing inside
fcoe_interface_cleanup to have log off before ndo_fcoe_disable
disables the fcoe.
This patch also fixes asymmetric rtnl locking around fcoe_if_destroy,
as currently this function requires rtnl held by its caller
and then have this func drops the lock, instead now don't have
any processing under rtnl inside fcoe_if_destroy, this required
moving few func to get build working again.

Added REC_TOV_CONST intent was to have rec tov as e_d_tov + 1s
but currently it is e_d_tov + 1ms since e_d_tov is stored in ms
unit.
Also returned rec tov by get_fsp_rec_tov is in ms and this ms tov
is used as-is with fc_fcp_timer_set expecting jiffies tov.
Fixed this by having get_fsp_rec_tov return rec tov in jiffies
as e_d_tov + 1s and then use jiffies tov w/ fc_fcp_timer_set.
Also some cleanup, no need to cache get_fsp_rec_tov return value
in local rec_tov at various places.

Reset using "fcoeadm -r" also needs to restart FIP before
doing libfc lport reset, this is needed for new switch firmware
requiring FIP solicitation before doing FLOGI again during reset.
So this patch does this by doing fcoe_ctlr_link_down and then
fcoe_ctlr_link_up to reset the interface.
The fcoe_ctlr_link_down call path also does lport reset
and then fcoe_ctlr_link_up re-starts the fabric login after
doing FIP solicitation first to get reset feature working
again.

Currently fsp->cmd is set to NULL w/o scsi_queue_lock before
dequeuing from scsi_pkt_queue and that could cause NULL
fsp->cmd in fc_fcp_cleanup_each_cmd for cmd completing
with fsp->cmd = NULL after fc_fcp_cleanup_each_cmd taken
reference. No need to set fsp->cmd to NULL as this is also
protected by fc_fcp_lock_pkt(), for above race the
fc_fcp_lock_pkt() in fc_fcp_cleanup_each_cmd() will fail
as that cmd is already done.

Sometimes switch in NPV mode rejects flogi request with DID
zero and in that case flogi is not tried again and port
remains offline, so this patch validates DID for non zero
along with only ACC response to allow flogi retry
for RJT with DID=0 also succeed FLOGI in next try.

This check prevents FCF selection in NPV mode due to zero fabric name
in that case and in turn flogi fails. Though NPV mode should not have
this zero and should be fixed there also but spec also does not require
initiator to ensure that fabric name must be non-zero, therefore dropping
this check to get flogi working in NPV mode.

This reverts commit cc0136c2e9.
That commit introduced vlan id info to WWPN but WWPN needs to
remain static as an unique port identifier in the fabric, therefore
variable fabric vlan id info doesn't need to be coded inside WWPN.
After this revert, port arg to fcoe_wwn_from_mac is always zero
but still leaving it as-is okay to later allow users to use NAA 2
scheme with this additional port arg.
Note with this patch, existing zoning using WWPN would require
re-zoning this time only and later no more re-zoning due to any
vlan id changes.

This is per FC-BB-5 Annex-D recommendation and per that
if address checking fails then drop the frame.
FIP code paths are already doing this so only needed for fcoe
frames.
The src address checking is limited to only fip mode since
this might break non-fip mode used in p2p due to used OUI
based addressing in some p2p code paths, going forward FIP
will be the only mode, therefore limited this to only FIP
mode so that it won't break non-fip p2p mode for now.

The fc_fabric_logoff and fc_fabric_login are redundant
here after recently added fcoe_ctlr_link_down/up to
these functions, therefore this patch removes logoff
and login to only use link down and up here. This works
best for their current usages with fcoe DCB link down or up.
This also works well to avoid EIO errors when fcoe DCB link
goes down as lport state moves out of ready quickly from
fcoe_ctlr_link_down and that allows re-queuing timed out IOs
for this case also.

In this case, sync IO fails with EIO(5) errors as:-
"Thread:1 System call error:5 - Input/output error (::pwrite() failed)".
This is due to IO time out while libfc doing link down processing
to block all rports and if timed out IO was at last retry
attempt then it fails to user with EIO error followed by
these log messages.

To fix these EIO errors, such timed out incomplete IOs needs

Currently fcoe module ref count is used for tracking
active fcoe instances, it means each fcoe instance create
increments the count while destroy dec the count.
The dec is done only if fcoe instance is destroyed from
/sysfs but not if destroyed due to NETDEV_UNREGISTER event.
So this patch moves only module_put doing dec to common
fcoe_if_destroy function, so that dec would occur on ever
fcoe instance destroy.

Currently rtnl mutex is grabbed during fcoe create, destroy, enable
and disable operations while sysfs s_active read mutex is already
held, but simultaneously other networking events could try grabbing
write s_active mutex while rtnl is already held and that is causing
circular lock warning, its detailed log pasted at end.
In this log, the rtnl was held before write s_active during device
renaming but there are more such cases as Joe reported another
instance with tg3 open at:-

This patch fixes this issue by not waiting for rtnl mutex during
fcoe ops, that means if rtnl mutex is not immediately available
then restart_syscall() to allow others waiting in line to
grab s_active along with rtnl mutex to finish their work first
under these mutex.
Currently rtnl mutex was grabbed twice during fcoe_destroy call flow,
second grab was from fcoe_if_destroy called from fcoe_destroy after
dropping rtnl mutex before calling fcoe_if_destroy, so instead made
fcoe_if_destroy always called with rtnl mutex held to have this mutex
grabbed only once in this code path.
However left matching rtnl_unlock as-is in its original place as it was
dropped there for good reason since very next call causes synchronous
fip worker flush and if rtnl mutex is still held before flush
then that would cause new circular warning between fip->recv_work and
rtnl mutex, I've added detailed comment for this on fcoe_if_destroy
calling and rtnl muxtes unlocking.

fcoe_create exits using out_nodev label when module is not
yet LIVE but this exit path unlocks the rtnl_lock though
rtnl lock was not held in this case.
So this patch replaces out_nodev with out_nomod to exit
w/o unlocking rtnl_lock.

No reason to restrict CDB size to 12 bytes in fcoe, so
increased to 16 so that 16 bytes SCSI CDB doesn't fail.
Uses common define to set max_cmd_len for fcoe and fnic,
fnic is already setting max_cmd_len to 16.
sg_readcap -l fails without this fix.

Currently fc_fcp_recv_data calls fc_fcp_retry_cmd to
retry failed IO but in this case tgt is still sending
data frames, therefore exchange needs to be aborted
first before initiating retry. So this patch fixes
this by aborting exchange first then have retry.
Renames fc_timeout_error to fc_fcp_recovery since
fc_timeout_error is already called from several other
places beside from fcp timeout handler and then
used fc_fcp_recovery for abort & retry from
fc_fcp_recv_data, this rename also required renaming
FC_CMD_TIME_OUT status to FC_CMD_RECOVERY to be
consistent with new fc_fcp_recovery.
Data frames are not expected for an DDPed exchange and
potentially it could be tampered data frame, so does
recovery in this case by calling fc_fcp_recovery.

Since use of offloads is more efficient than switching
to non-offload EM. However kept logic same to call em_match
if it is provided in the list of EMs.

In some cases seq is incremented twice causing unnecessary
seq jump, for instance fc_exch_recv_seq_resp increments
seq id when fc_sof_is_init is true and that is true for
each incoming xfer ready but then fc_fcp_send_data does
another seq increment to send data for xfer ready.
This patch removes all such seq id jumps, at least it
eliminates few calls to fc_seq_start_next using ex_lock.
Also removes seq id update with incoming frame's seq id
as this is not needed since each end (I or T) just need
to send incremented their own seq id on each TSI from
other end & before sending new sequence within a
exchange.

This is required to correctly select vlan tx queue for a driver
supporting multi tx queue with ndo_select_queue implemented since
currently selected vlan tx queue is unaligned to selected queue by
real net_devce ndo_select_queue.
Unaligned vlan tx queue selection causes thrash with higher vlan
tx lock contention for least fcoe traffic and wrong socket tx
queue_mapping for ixgbe having ndo_select_queue implemented.

Currently netdev_features_change is called before fcoe tx queues
setup is done, so this patch moves calling of netdev_features_change
after tx queues setup is done in ixgbe_init_interrupt_scheme, so
that real_num_tx_queues is updated correctly on each fcoe enable
or disable.
This allows additional fcoe queues updated correctly in vlan driver
for their correct queue selection.

Adds check to call fc_fcp_ddp_setup for only FCP read cmds to avoid
accessing junk fsp pointer at least in ESX since non FCP frame had
junk fsp value, though fsp is implicitly initialized to null
by __alloc_skb but with this patch no more relying on fsp
initialized to null value and hitting junk fsp ptr access.
Removes fsp pointer checking in fc_fcp_ddp_setup as this is not
needed any more since its only caller for FCP read will always
have a valid fsp.

This is to allow fcoemon util to enable or disable a fcoe interface
according to DCB link state change.
Adds sysfs module param enable and disable for this and also
updates existing other module param description to be consistent
and more accurate since older description had double "fcoe" word
with less meaningful netdev reference to user space.
Adds code to ignore redundant fc_lport_enter_reset handling for a
already disabled fcoe interface by checking LPORT_ST_DISABLED
or LPORT_ST_LOGO states, this also prevents lport state transition
on link flap on a disabled interface.
Above changes required lport state transition to get out of
disabled or logo state on call to fc_fabric_login.

scheduled delayed work after EM mempool is destroyed causing

This patch fixes this issue by adding dedicated work queue thread
fc_exch_workqueue for exch delayed work and then flush this work
queue before destroying EM mempool.
The cancel_delayed_work_sync cannot be called during final
fc_exch_reset due to lport and exch locking ordering, so removes
related comment block not relevant any more with this patch.

Adds last_can_queue_ramp_down_time and updates this on every
ramp down. If last_can_queue_ramp_down_time is not zero then
do ramp up on any IO completion in added fc_fcp_can_queue_ramp_up.
Reset last_can_queue_ramp_down_time to zero once can_queue
is ramped up to added max_can_queue limit, this is to avoid any
more ramp up attempts on subsequent IO completion.
The ramp down and up are skipped for FC_CAN_QUEUE_PERIOD
to avoid infrequent changes to can_queue, this required
keeping track of ramp up time also in last_can_queue_ramp_up_time.
Adds code to ramp down can_queue if lp->qfull is set, with added
new ramp up code the can_queue will be increased after
FC_CAN_QUEUE_PERIOD, therefore it is safe to do ramp down
without fsp in this case and will avoid thrash. This required
fc_fcp_can_queue_ramp_down locking change so that it can be
called with Scsi_Host lock held.
Removes si->throttled and fsp state FC_SRB_NOMEM, not needed with
added ramp up code.

Currently can_queue is reduced only if frame alloc fails
during fc_fcp_send_data but frame alloc can fail at several
other places in FCP data path and can_queue needs to be
reduced for any FCP frame alloc failure.
This patch adds fc_fcp_frame_alloc for all FCP frame allocations
and if fc_frame_alloc fails in fc_fcp_frame_alloc then reduce
can_queue in fc_fcp_frame_alloc, this will reduce can_queue for
all FCP frame alloc failures.
This required moving fc_fcp_reduce_can_queue up, to build without
adding its prototype. Also renamed fc_fcp_reduce_can_queue to
fc_fcp_can_queue_ramp_down.
Removes fc_fcp_reduce_can_queue calling from fc_fcp_recv since
not needed with added fc_fcp_frame_alloc reducing can_queue.

Cleans up frame allocation APIs to have just single fc_frame_alloc API.
Removes _fc_frame_alloc, renames __fc_frame_alloc to _fc_frame_alloc.
Modifies fc_fcp_send_data for removed _fc_frame_alloc, fc_fcp_send_data
was the only user of removed _fc_frame_alloc.
Also Adds check in fc_frame_alloc to do mod by 4 for only non-zero
len value.
This patch is prep work to fix can_queue reducing in next patch.
Single fc_frame_alloc API helps in fixing can_queue reducing in
next patch.

Current FC HBA queue_depth ramp up code depends on last queue
full time. The sdev already has last_queue_full_time field to
track last queue full time but stored value is truncated by
last four bits.
So this patch updates last_queue_full_time without truncating
last 4 bits to store full value and then updates its only
current usages in scsi_track_queue_full to ignore last four bits
to keep current usages same while also use this field
in added ramp up code.
Adds scsi_handle_queue_ramp_up to ramp up queue_depth on
successful completion of IO. The scsi_handle_queue_ramp_up will
do ramp up on all luns of a target, just same as ramp down done
on all luns on a target.
The ramp up is skipped in case the change_queue_depth is not
supported by LLD or already reached to added max_queue_depth.
Updates added max_queue_depth on every new update to default
queue_depth value.
The ramp up is also skipped if lapsed time since either last
queue ramp up or down is less than LLD specified
queue_ramp_up_period.
Adds queue_ramp_up_period to sysfs but only if change_queue_depth
is supported since ramp up and queue_ramp_up_period is needed only
in case change_queue_depth is supported first.
Initializes queue_ramp_up_period to 120HZ jiffies as initial
default value, it is same as used in existing lpfc and qla2xxx.

The cmd_per_lun value is used by scsi-ml as fall back lowest
queue_depth value but in case of libfc cmd_per_lun is set to
same value as max queue_depth = 32.
So this patch reduces cmd_per_lun value to 3 and configures
each lun with default max queue_depth 32 in fc_slave_alloc.

Initializing these libfc globals per lport could mess up exch
allocation/free for existing lport.
So this patch moves their initialization to fc_setup_exch_mgr
so that these globals gets initialized only once for libfc.

1. Updates fcoe_rcv() to queue incoming frames to the fcoe per
cpu thread on which this frame's exch was originated and simply
use current cpu for request exch not originated by initiator.
It is redundant to add this code under CONFIG_SMP, so removes
CONFIG_SMP uses around this code.
2. Updates fc_exch_em_alloc, fc_exch_delete, fc_exch_find to use
per cpu exch pools, here fc_exch_delete is rename of older
fc_exch_mgr_delete_ep since ep/exch are now deleted in pools
of EM and so brief new name is sufficient and better name.
Updates these functions to map exch id to their index into exch
pool using fc_cpu_mask, fc_cpu_order and EM min_xid.
This mapping is as per detailed explanation about this in
last patch and basically this is just as lower fc_cpu_mask
bits of exch id as cpu number and upper bit sum of EM min_xid
and exch index in pool.
Uses pool next_index to keep track of exch allocation from
pool along with pool_max_index as upper bound of exches array
in pool.
3. Adds exch pool ptr to fc_exch to free exch to its pool in
fc_exch_delete.
4. Updates fc_exch_mgr_reset to reset all exch pools of an EM,
this required adding fc_exch_pool_reset func to reset exches
in pool and then have fc_exch_mgr_reset call fc_exch_pool_reset
for each pool within each EM for a lport.
5. Removes no longer needed exches array, em_lock, next_xid, and
total_exches from struct fc_exch_mgr, these are not needed after
use of per cpu exch pool, also removes not used max_read,
last_read from struct fc_exch_mgr.
6. Updates locking notes for exch pool lock with fc_exch lock and
uses pool lock in exch allocation, lookup and reset.

Adds per cpu exch pool for these reasons:-
1. Currently an EM instance is shared across all cpus to manage
all exches for all cpus. This required em_lock across all
cpus for an exch alloc, free, lookup and reset each frame
and that made em_lock expensive, so instead having per cpu
exch pool with their own per cpu pool lock will likely reduce
locking contention in fast path for an exch alloc, free and
lookup.
2. Per cpu exch pool will likely improve cache hit ratio since
all frames of an exch will be processed on the same cpu on
which exch originated.
This patch is only prep work to help in keeping complexity of next
patch low, so this patch only sets up per cpu exch pool and related
helper funcs to be used by next patch. The next patch fully makes
use of per cpu exch pool in all code paths ie. tx, rx and reset.
Divides per EM exch id range equally across all cpus to setup per
cpu exch pool. This division is such that lower bits of exch id
carries cpu number info on which exch originated, later a simple
bitwise AND operation on exch id of incoming frame with fc_cpu_mask
retrieves cpu number info to direct all frames to same cpu on which
exch originated. This required a global fc_cpu_mask and fc_cpu_order
initialized to max possible cpus number nr_cpu_ids rounded up to 2's
power, this will be used in mapping exch id and exch ptr array
index in pool during exch allocation, find or reset code paths.
Adds a check in fc_exch_mgr_alloc() to ensure specified min_xid
lower bits are zero since these bits are used to carry cpu info.
Adds and initializes struct fc_exch_pool with all required fields
to manage exches in pool.
Allocates per cpu struct fc_exch_pool with memory for exches array
for range of exches per pool. The exches array memory is followed
by struct fc_exch_pool.
Adds fc_exch_ptr_get/set() helper functions to get/set exch ptr in
pool exches array at specified array index.
Increases default FCOE_MAX_XID to 0x0FFF from 0x07EF, so that more
exches are available per cpu after above described exch id range
division across all cpus to each pool.

The phys_dev was used only to locate common offload EM instance for all
FCoE instances on a eth devices in function fcoe_em_config, so just updated
fcoe_em_config to look for actual real eth device in locating common offload
EM instance and then no need to store phys_dev in fcoe_softc, so removes
phys_dev from fcoe_softc also.
Renames fcoe_softc real_dev to netdev and updates all its uses to use netdev.
So effectively no functional change, use of single netdev instead phys_dev
and real_dev saves one pointer memory in fcoe_softc, also real_dev used here
was confusing with vlan driver terminology since real_dev in vlan driver is
referred to physical eth device.

Updates fcoe_em_config to allocate a single instance of sharable offload
EM for supported lp->lro_xid per eth device, and then share this EM
for subsequently more lports creation on same eth device (e.g when using
VLAN).
Adds tiny fcoe_oem_match function for offload EM to return true for read
types IO to have read IO exchanges allocated from offload shared EM.
Removes fc_em_alloc_xid function completely which was needed to manage
two xid ranges within a EM, this is not needed any more with allocation
of separate sharable offload EM per eth device. Instead this patch adds
simple xid allocation logic to manage single xid range.
Adds fc_exch_em_alloc with mp->next_xid as cursor to allocate new xid
from single xid range of EM, uses mp->next_xid instead removed mp->last_xid
which slightly increase probability of finding empty xid on exch allocation.
Removes restriction of not allowing use of xid zero along with changing
two xid range change to single xid range.
Makes fc_fcp_ddp_setup calling conditional to only xid allocated from
shared offload EM.

Modifies fcoe_hostlist_lock uses such that a new EM allocation in
fcoe_em_config and adding new fcoe_softc using fcoe_hostlist_add
are atomic, this is to ensure that a shared offload EM gets allocated
only once per eth device for its all lports.

Modifies current code to use EM anchor list in EM allocation, EM free,
EM reset, exch allocation and exch lookup code paths.
1. Modifies fc_exch_mgr_alloc to accept EM match function and then
have allocated EM added to the lport using fc_exch_mgr_add API
while also updating EM kref for newly added EM.
2. Updates fc_exch_mgr_free API to accept only lport pointer instead
EM and then have this API free all EMs of the lport from EM anchor
list.
3. Removes single lport pointer link from the EM, which was used in
associating lport pointer in newly allocated exchange. Instead have
lport pointer passed along new exchange allocation call path and
then store passed lport pointer in newly allocated exchange, this
will allow a single EM instance to be used across more than one
lport and used in EM reset to reset only lport specific exchanges.
4. Modifies fc_exch_mgr_reset to reset all EMs from the EM anchor list
of the lport, adds additional exch lport pointer (ep->lp) check for
shared EM case to reset exchange specific to a lport requested reset.
5. Updates exch allocation API fc_exch_alloc to use EM anchor list and
its anchor match func pointer. The fc_exch_alloc will walk the list
of EMs until it finds a match, a match will be either null match
func pointer or call to match function returning true value.
6. Updates fc_exch_recv to accept incoming frame on local port using
only lport pointer and frame pointer without specifying EM instance
of incoming frame. Instead modified fc_exch_recv to locate EM for the
incoming frame by matching xid of incoming frame against a EM xid range.
This change was required to use EM list in libfc Rx path and after this
change the lport fc_exch_mgr pointer emp is not needed anymore, so
removed emp pointer.
7. Updates fnic for removed lport emp pointer and above modified libfc APIs
fc_exch_recv, fc_exch_mgr_alloc and fc_exch_mgr_free.
8. Removes exch_get and exch_put from libfc_function_template as these
are no longer needed with EM anchor list and its match function use.
Also removes its default function fc_exch_get.
A defect this patch introduced regarding the libfc initialization order in
the fnic driver was fixed by Joe Eykholt <jeykholt@cisco.com>.

Adds EM list using a anchor struct fc_exch_mgr_anchor, anchor is used
to allow same EM instance sharing across more than one lport on a eth
device, this implementation is per discussed design posted at
http://www.open-fcoe.org/pipermail/devel/2009-June/002566.html.
The shared EM is required for multiple lports on eth device when
using multiple VLANs or NPIV.
Adds fc_exch_mgr_add API to add a EM to the lport and fc_exch_mgr_del
API to delete previously added EM.
Also adds function fc_exch_mgr_destroy() to destroy allocated EM.
The kref is added to the EM to keep track of EM usage count, the EM is
destroyed when no longer in use upon kref reaching to zero.
The caller can specify match function to fc_exch_mgr_add, this
will be used in determining exchange allocation from its EM or not.
Moved calling of fcoe_em_config below fcoe_libfc_config calling,
so that list head lp->ema_list is initialized before configuring
EM.

Adds fcoe offload related net_device_ops functions vlan_dev_fcoe_ddp_setup
and vlan_dev_fcoe_ddp_done, their implementation simply calls real eth device
net_device_ops for FCoE DDP setup and done operations.
Updates VLAN netdev field value for fcoe_ddp_xid from real eth device netdev.
Above changes are required for fcoe DDP offload working on a VLAN interface.

Currently the fc_exch_rrq is called with fc_exch's ex_lock held.
The fc_exch_rrq allocates new exch and that requires taking
ex_lock again after EM lock. This locking order causes warning,
see more details on this warning at :-

This patch fixes this by dropping the ex_lock before calling
fc_exch_rrq().
The fc_exch_rrq needs to grab ex_lock lock again to schedule
RRQ retry and in the meanwhile fc_exch_reset could occur before
ex_lock is grabbed inside fc_exch_rrq. So to handle this case,
this patch adds additional check to detect fc_exch_reset after
ex_lock acquired and in case the fc_exch_reset occurred then
abandons the RRQ retry and releases the exch.

Removes periodic fcoe_watchdog timer used across all fcoe interface
maintained in fcoe_hostlist instead added new fcoe_queue_timer
per fcoe interface.
Added timer is armed only when some pending skb need to be flushed
as oppose to periodic 1 second fcoe_watchdog, since now
fcoe_queue_timer is used on demand thus set this to 2 jiffies.
Now fcoe_queue_timer is much simple than fcoe_watchdog using lock to
process all fcoe interface from fcoe_hostlist.
I noticed +ve performance result with using 2 jiffies timer as
this helps flushing fcoe_pending_queue quickly.

Currently fcoe_pending_queue.lock held twice for every new skb
adding to this queue when already least one pkt is pending in this
queue and that is not uncommon once skb pkts starts getting queued
here upon fcoe_start_io => dev_queue_xmit failure.
This patch moves most fcoe_pending_queue logic to fcoe_check_wait_queue
function, this new logic grabs fcoe_pending_queue.lock only once to
add a new skb instead twice as used to be.
I think after this patch call flow around fcoe_check_wait_queue
calling in fcoe_xmit is bit simplified with modified
fcoe_check_wait_queue function taking care of adding and
removing pending skb in one function.

If we can find a type NETDEV_HW_ADDR_T_SAN mac address from the
corresponding netdev for a fcoe interface then sets up added the
fc->ctlr.spma flag and stores spma mode address in ctl_src_addr.
In case the spma flag is set then:-
1. Adds spma mode MAC address in ctl_src_addr as secondary
MAC address, the FLOGI for FIP and pre-FIP will go out
using this address.
2. Cleans up stored spma MAC address in ctl_src_addr in
fcoe_netdev_cleanup.
3. Sets up spma bit in fip_flags for FIP solicitations along
with exiting FPMA bit setting.
4. Initialize the FLOGI FIP MAC descriptor to stored spma
MAC address in ctl_src_addr. This is used as proposed
FCoE MAC address from initiator along with both SPMA
and FPMA bit set in FIP solicitation, in response the
switch may grant any FPMA or SPMA mode MAC address to
initiator.
Removes FIP descriptor type checking against ELS type
ELS_FLOGI in fcoe_ctlr_encaps to update a FIP MAC descriptor,
instead now checks against FIP_DT_FLOGI.
I've tested this with available FPMA-only FCoE switch but
since data_src_addr is updated using same old code for
both FPMA and SPMA modes with FIP or pre-FIP links, so added
SPMA mode will work with SPMA-only switch also provided that
switch grants a valid MAC address.

Currently fcoe_netdev_config adds netdev pkt handler for fcoe pkts,
fcoe_if_create adds netdev pkt handler for fip packets, a secondary
MAC address is added by fcoe_netdev_config and then later cleanup
for these netdev related config/adds is done only during
fcoe_if_destroy and no cleanup done on error during fcoe interface
creation after above netdev config calling in fcoe_if_create.
So this patch adds single func for above mentioned cleanup the
fcoe_netdev_cleanup and then calls this func on either fcoe interface
destroy or exiting from fcoe_if_create due to an error after fcoe/fip
related above netdev config is done.
Moved netdev pkt handler addition code blocks for fip pkts close to
similar code block for foce pkt in fcoe_netdev_config, so that added
fcoe_netdev_cleanup could be called on error from fcoe_netdev_config
to undo these both additions for fcoe/fip pkt handlers. This move
required reference to fcoe_fip_recv in fcoe_netdev_config, so moved
fip related functions fcoe_fip_recv, fcoe_fip_send and
fcoe_update_src_mac above fcoe_netdev_config.
This consolidation will enable spma mode support in next patch to
easily add or delete spma mode mac address beside fixing current
no cleanup issue during error.
