Ivo van Doorn 4268d8ed64 rt2x00: Fix transfer speed regression for USB hardware
Patch:
  rt2x00: Make rt2x00_queue_entry_for_each more flexible
  commit: 10e11568ca

introduced a severe regression on the throughput
for USB hardware. It turns out that the exiting of
the rt2x00queue_for_each_entry() was done too early.

The exact cause for this regression is unknown,
but by disabling the premature exiting of the loop
seems to resolve the issue.

Ivo van Doorn 1c0bcf89d8 rt2x00: Add autowake support for USB hardware
The USB drivers don't support automatically waking up when in powersaving mode,
add a work object which will wakeup the device in time to receive the next beacon.
Based on that beacon, we either go back into powersaving mode, or we remain awake
to receive the buffered frames for our station.

Some part of the code, especially rt2x00lib_find_ie and rt2x00lib_rxdone_check_ps
are inspired on the code from carl9170.

Ivo van Doorn a073fdef46 rt2x00: Optimize TX_STA_FIFO register reading
Add recycling functionality to rt2x00usb_register_read_async.
When the callback function returns true, resubmit the urb to
read the register again.

This optimizes the rt2800usb driver when multiple TX status reports
are pending in the register, because now we don't need to allocate
the rt2x00_async_read_data and urb structure each time.

When no TX status was available, the default timeout
of 20ms is a bit high. The frame is highly likely already
send out, so the TX status should be available within
only a few milliseconds.

Ivo van Doorn 0ed7b3c044 rt2x00: Implement get_antenna and set_antenna callback functions
Implement the get_antenna and set_antenna callback functions, which will
allow clients to control the antenna for all non-11n hardware (Antenna handling
in rt2800 is still a bit magical, so we can't use the set_antenna for those drivers
yet).

To best support the set_antenna callback some modifications are needed in the
diversity handling. We should never look at the default antenna settings to determine
if software diversity is enabled. Instead we should set the diversity flag when
possible, which will allow the link_tuner to automatically pick up the tuning.

Ivo van Doorn 8d0a2dcfb6 rt2x00: Optimize register access in rt2800usb
All register reads/writes in rt2800usb were previously done with
rt2800_register_read/rt2800_register_write. These however indirectly
call rt2x00usb_register_read/rt2x00usb_register_write which adds an
additional overhead of at least one call and several move instructions
to each register access.

Replacing the calls to rt2800_register_read/rt2800_register_write with
direct calls to rt2x00usb_register_read/rt2x00usb_register_write gets
rid of quite a number of instructions in the drivers hotpaths (IRQ
handling and txdone handling).

For consistency replace all references to rt2800_register_read/write
with the rt2x00usb_register_read/write variants.

Ivo van Doorn 152a599274 rt2x00: Decrease association time for USB devices
When powersaving is enabled, assocaition times are very high
(for WPA2 networks, the time can easily be around the 3 seconds).

This is caused, because the flushing of the queues takes
too much time. Without the flushing callback mac80211 assumes
a timeout of 100ms while scanning. Limit all flush waiting
loops to the same maximum.

We can apply this maximum by passing the drop status to the
driver, which makes sure the driver performs extra actions
during the waiting for the queue to become empty.

After these changes, association times fall within the
healthy range of ~0.6 seconds with powersaving enabled.
The difference between association time between powersaving
enabled and disabled is now only ~0.1 second (which can also
be due to the measuring method).

Ivo van Doorn 7dab73b37f rt2x00: Split rt2x00dev->flags
The number of flags defined for the rt2x00dev->flags field,
has been growing over the years. Currently we are approaching
the maximum number of bits which are available in the field.

A secondary problem, is that one part of the field are initialized only
during boot, because the driver requirements are initialized or device
requirements are loaded from the EEPROM. In both cases, the flags are
fixed and will not change during device operation. The other flags are
the device state, and will change frequently. So far this resulted in the fact
that for some flags, the atomic bit accessors are used, while for the others
the non-atomic variants are used.

By splitting the flags up into a "flags" and "cap_flags" we can put all flags
which are fixed inside "cap_flags". This field can then be read non-atomically.
In the "flags" field we keep the device state, which is going to be read atomically.

This adds more room for more flags in the future, and sanitizes the field access methods.

Ivo van Doorn 0439f5367c rt2x00: Move TX/RX work into dedicated workqueue
The TX/RX work structures must be able to run independently
of other workqueues. This is because mac80211 might use
the flush() callback function from various context, which depends
on the TX/RX work to complete while the main thread is blocked
(until the the TX queues are empty).

This should reduce the number of  'Queue %d failed to flush' warnings.

Ivo van Doorn f615e9a38a rt2x00: Fix WMM Queue naming
The Queue names were incorrectly copied from the legacy drivers,
as a result the queue names were inversed to what was expected.

This renames the queues using this mapping:
	QID_AC_BK -> QID_AC_VO (priority 0)
	QID_AC_BE -> QID_AC_VI (priority 1)
	QID_AC_VI -> QID_AC_BE (priority 2)
	QID_AC_VO -> QID_AC_BK (priority 3)

Note that this was a naming problem only, which didn't affect
the assignment of frames to their respective queues.

Ivo van Doorn 64e7d72384 rt2x00: Cleanup RX index counting
Add the rt2x00_dmastart function to rt2x00lib which
marks the queue_entry as "owned by device", and increased
the Q_INDEX number.

This cleanups up the index handling by rt2x00lib which
at until so far used hackish approaches to keep the
RX queue index numbering sane.

Queues can also be paused which will halt the flow

As part of the queue refactoring, change the queue callback
function names to have 3 different actions: start, kick & stop.

We can now also remove the STATE_RADIO_RX_ON/STATE_RADIO_RX_OFF
device_state flags, and replace the usage with using the
start_queue/stop_queue callback functions.
This streamlines the RX queue handling to the
similar approach as all other queues.

Ivo van Doorn 5450b7e2f0 rt2x00: Introduce 3 queue commands in drivers (start, kick, stop).
As part of the queue refactoring, we now introduce
3 queue commands: start, kick, stop.

 - Start: will enable a queue, for TX this will
   not mean anything, while for beacons and RX
   this will update the registers to enable the queue.
 - Kick: This will kick all pending frames to
   the hardware. This is needed for the TX queue
   to push all frames to the HW after the queue
   has been started
 - Stop: This will stop the queue in the hardware,
   and cancel any pending work (So this doesn't
   mean the queue is empty after a stop!).

Move all code from the drivers into the appropriate
functions, and link those calls to the old rt2x00lib
callback functions (we will fix this later when we
refactor the queue control inside rt2x00lib).

Ivo van Doorn ae4ecb9f8f rt2x00: Increase REGISTER_BUSY_COUNT
For some hardware the REGISTER_BUSY_COUNT isn't sufficient,
increase the REGISTER_BUSY_COUNT to 100 to catch most
devices which have more problems with accessing the registers.

For normal operating devices nothing would change as they will
exit the loop early anyway.

Ivo van Doorn 8c5765fda4 rt2x00: Add watchdog functions for HW queue
Add watchdog functions for managing the Queues inside the hardware.
Normally the driver doesn't have much to do with these queues
directly, but the Ralink drivers did implement watchdog functions
for these. These watchdog functions are not triggered that often,
compared to the other watchdog functions, but I have at least
seen them trigger once or twice during a long stresstest run.

can be removed and replaced with the call directly to
the set_device_state callback function.

We can remove the STATE_RADIO_RX_ON_LINK and

special behavior inside rt2x00lib rather then the
drivers.

By not scheduling the TX/RX completion worker threads
when Radio is disabled, or hardware has been unplugged,
the queues cannot be completely cleaned.

This causes crashes when the hardware has been unplugged while
the radio is still enabled.

Ivo van Doorn f44df18c58 rt2x00: Implement flush callback
Implement a basic flush callback function, which simply loops
over all TX queues and waits until all frames have been transmitted
and the status reports have been gathered.

At this moment we don't support dropping any frames during the
flush, but mac80211 will only send 'false' for this argument anyway,
so this is not important at this time.

Ivo van Doorn aaf886bd21 rt2x00: Remove failsave from rt2x00usb_watchdog_tx_dma
When the TX status handler failed to clear the queue
in rt2x00usb_watchdog_tx_dma() we shouldn't use a failsave
to use the rt2x00usb txdone handler.

If a driver has overriden the txdone handler it must make
sure the txdone handler is capable of cleaning up the queue itself.

Ivo van Doorn a13c8f3133 rt2x00: Fix URB error handling
kill_urb guarentees that when the function returns, the URB has
been fully killed. This means we don't need the extra sleeping
after the call to kill_urb.

kill_urb can however also guarentee the submit_urb to fail, as
a result, we must catch the return value from submit_urb an
correctly mark the entry as owned by the driver, and the
status as broken.

Ivo van Doorn cdfd2c5cff rt2x00: Move watchdog work to kernel work_queue
The watchdog function must run on a work_queue
which is independent of any other work inside rt2x00.

The main reasons, being that a broken work on the mac80211
work_queue can otherwise prevent the watchdog to run (while
in fact the watchdog could fix the issue). And on the other
hand because the watchdog relies on the completion of the
completion handlers for RX/TX which for the USB case, occur
on the mac80211 workqueue.

This fixes some "Queue %d failed to flush" errors, which were
caused by the watchdog function waiting on the completion
handler which was scheduled to run right after the watchdog.

A lot of functions accept a struct rt2x00_dev combined with
either a struct queue_entry or struct data_queue argument.
This can be simplified by only passing on the queue/entry
argument.

In cases where rt2x00_dev and a sk_buff are send together,
we can send the queue_entry instead.

rt2x00usb_alloc_urb and rt2x00usb_free_urb have a bit
of vague naming. Instead they allocate all the data which
belongs to a rt2x00 data queue entry.

Ivo van Doorn bc8a979e2d rt2x00: Improve TX status entry validation
The TX_STA_FIFO contains some information for identifying
a outgoing frame, however matching by WCID and ACK status is
not sufficient to 100% identify the macthing queue_entry structure
(containing the SKB buffer) which belongs to the status report.

Within TX_STA_FIFO we have a 4-bit field named PACKETID, which is
currently used to encode the queue id. The queue ID is however
limited to values from 0 to 3, which means 2 bits are sufficient
to encode the value. With the remaining 2 bits we can encode a
partial queue_entry index number. The value of PACKETID is not
allowed to become 0, with the queue ID ranging from 0 to 3, at least
one of the bits for the entry identification must be 1.

That leaves us with 3 possible values we can still encode in the
bits. Altough this doesn't allow 100% accurate matching of the
TX_STA_FIFO queue to a queue_entry structure, it at least improves
the accuracy. This allows us to better detect if we have missed the
TX_STA_FIFO report, which in turn reduces the number of watchdog
warnings regarding the TX status timeout.

Ivo van Doorn cd35a39121 rt2x00: Cleanup rt2x00usb_watchdog_reset_tx
rt2x00usb_watchdog_reset_tx performs the same task
as rt2x00usb_kill_tx_queue, with the only difference
is that it waits for all entries to be returned to
the driver and for all frames the status has been
reported to mac80211.

We can easily split this task by calling rt2x00usb_kill_tx_queue,
sleep for a short period and invoke the TX status reporting
function. By adding the sleep() to the kill_entry we make sure
that even during shutdown we guarentee the entry has been killed when
the function returns. To make this work correctly the interrupt
handlers have to be updated to prevent checking for the RADIO_ENABLED
flag too early which prevents the ownership of the entry to be reset.
Additionally a check for the DEVICE_PRESENT flag is not required but
is nice to prevent race conditions when the device was unplugged.

Additionally rather then calling rt2x00usb_work_txdone() for
status reporting we let the driver perform the TX status reporting
first. If this is not sufficient then rt2x00usb_work_txdone() will
still be used to cleanup the mess.

Ivo van Doorn 652a9dd2a0 rt2x00: Split watchdog check into a DMA and STATUS timeout
The watchdog for rt2800usb triggers frequently causing all URB's
to be canceled often enough to interrupt the normal TX flow.
More research indicated that not the URB upload to the USB host
were hanging, but instead the TX status reports.

To correctly detect what is going on, we introduce Q_INDEX_DMA_DONE
which is an index counter between Q_INDEX_DONE and Q_INDEX and indicates
if the frame has been transfered to the device.

This also requires the rt2x00queue timeout functions to be updated
to differentiate between a DMA timeout (time between Q_INDEX and
Q_INDEX_DMA_DONE timeout) and a STATUS timeout (time between
Q_INDEX_DMA_DONE and Q_INDEX_DONE timeout)

All Q_INDEX_DMA_DONE code was taken from the RFC from
Helmut Schaa <helmut.schaa@googlemail.com> for the implementation
for watchdog for rt2800pci.

USB devices upload their beacon and then automatically send
it out every beacon interval. However when killing a TX queue
we only kill the URB and not the actual transmission of the beacon.

This will reset the Beacon register to prevent any beacons from

Ivo van Doorn 3613884d96 rt2x00: Validate TX status results with current data entry
Instead of printing a warning when the PID, ACK, or WCID of
an entry don't match the TX status report, we should skip the
entry to search for the entry which actually does match
the TX status data.

This reduces the number of watchdog errors on the TX queues
for rt2800usb, and seems to improve the reliability of the
TX flow a bit.

Ivo van Doorn 8d1331b37d rt2x00: Fix max TX power settings
During initialization each driver reads the default TX power
for each individual channel. However mac80211 only accepts the
maximum value (which is also handled as default value).

As a result, the TX power of the device was being limited to
the default value, which is often quite low compared to the
real maximum acceptable value.

This patch allows each driver to set the maximum value on a
per-channel basis which is forwarded to mac80211. The default
value will be preserved for now, in case we want to update
mac80211 to differentiate between the maximum and default txpower.

This fixes bug complaining about limited TX power values like:
https://bugzilla.kernel.org/show_bug.cgi?id=16358

The retry count for a frame is calculated by the realized MCS rate
subtracting of the requested MCS rate. However during TX done reporting
we accidently override the requested MCS rate with the realized MCS
rate. This causes the retry count to be reset to 0 always, and feeding
invalid rate information into minstrel_ht.

Ivo van Doorn 5eb7efe8a4 rt2x00: Move direct access to queue->entries to rt2x00queue.c
All access to queue->entries through the Q_INDEX/Q_INDEX_DONE
variables must be done using spinlock protection. It is best
to manage this completely from rt2x00queue.c.

For safely looping through all entries in the queue, the function
rt2x00queue_for_each_entry is added which will walk from from a index
range in a safe manner.

This also fixes rt2x00usb which walked the entries list from
0 to length to kill each entry (killing entries must be done
from Q_INDEX_DONE to Q_INDEX to enforce TX status reporting to
occur in the correct order.

Ivo van Doorn 933314582e rt2x00: Simplify arguments to rt2x00 driver callback functions
write_tx_desc shouldn't pass a rt2x00dev and skb pointer,
instead it should use the same format as other TX frame
callback functions, which is passing the data_entry pointer
which contains all the information which is needed to work
on a TX frame.

Most callers of the kick_tx_queue and kill_tx_queue already
have the data_queue pointer, so rather then sending the QID
with the given function, when the driver requests a new
pointer to the data_queue, it is more efficient to just
send the data_queue pointer directly.

Ivo van Doorn e5ef5bad34 rt2x00: Remove ieee80211_rx_status from rt2x00_dev
rt2x00 was keeping a copy of ieee80211_rx_status embedded
into the rt2x00_dev structure. For each RX frame, this structure
was copied into the skb->cb where mac80211 would handle it further.

However at the moment only the fields current band, and frequency
were updated. Whereas the band was already provided directly within
the rt2x00_dev structure. Save a memcpy action, and reduce memory
a bit, by adding a curr_freq field to rt2x00_dev, and completely
remove the ieee80211_rx_status structure from rt2x00_dev.

The TX_STA_FIFO register which is used for per-frame TX frame
status reporting is also valid on rt2800usb. We can move the
rt2800pci_txdone function into rt2800lib where it can also
be used by rt2800usb.

rt2800usb needs to overwrite the txdone work handler to
a different function.

Both rt2800usb as rt2800_txdone need to take into account
that IO failures can occur while uploading the URB, which
means that when obtaining the new entry the IO status must
be checked.

The only reason why the write_tx_data callback function
is needed inside the driver, is because the location of
the TXWI descriptor is different on PCI and USB hardware.
Except for the beacon, where the TXWI is always at the
start of the SKB buffer.

In both cases the drivers write_tx_data function only
wrap around the function rt2800_write_txwi. Move write_tx_data
completely into the rt2800lib library, and add a callback
function to obtain the TXWI pointer.

Ivo van Doorn 7e613e1666 rt2x00: Move USB tx/rx done handling to workqueue
Move all TX and RX completion handling into a work structure,
which is handeled on the mac80211 workqueue. This simplifies
the code in rt2x00lib since it no longer needs to check if the
device is USB or PCI to decide which mac80211 function should be used.

In the watchdog some changes are needed since it can no longer rely
on the TX completion function to be run while looping through the
entries. (Both functions now work on the same workqueue, so this
would deadlock). So the watchdog now waits for the URB to return,
and handle the TX status report directly.

As a side-effect, the debugfs entry for the RX queue now correctly
displays the positions of the INDEX and INDEX_DONE counters. This
also implies that it is not possible to perform checks like queue_empty()
and queue_full() on the RX queue.

Implement watchdog monitoring for USB devices (PCI support can
be added later). This will determine if URBs being uploaded to
the hardware are actually returning. Both rt2500usb and rt2800usb
have shown that URBs being uploaded can remain hanging without
being released by the hardware.
By using this watchdog, a queue can be reset when this occurs.
For rt2800usb it has been tested that the connection is preserved
even though this interruption.

Currently vgc_level_reg and vgc_level are equal to eachother,
while the purpose of vgc_level_reg is the value last written
to the register and is remembered through link tuning resets.
The vgc_level is the currently active level, which is
reset during link tuning resets.

The usage of these variables depends on the drivers, some drivers
need both, while others need only one of the two.

Ivo van Doorn 7486192206 rt2x00: Convert AGC value from descriptor to RSSI (dBm)
The RSSI values in the RXWI descriptor aren't true RSSI
values. Instead they are more like the AGC values similar
to rt61pci. And as such, it needs the same conversion
before it can be passed to rt2x00lib/mac80211.

This requires the struct queue_entry to be passed to
rt2800_process_rxwi rather then the skb structure which
is contained in the queue_entry. This is required to
obtain the lna_gain information from the rt2x00_dev structure.

This fixes connection problems when using wpa_supplicant
which would try to connect to the worst AP's rather then the
best ones.

Legacy driver indicates that BBP1_TX_ANTENNA must be set
to 0 for TXPATH values of 1 and 3. So the previous statement
that nothing should be done for TXPATH = 3, is false.

Furthermore, remove the false BBP3_RX_ANTENNA initialization
when TXPATH is 1 for PCI and SOC devices. This field will always
be overridden in the next switch statement, making this initialization
bogus. History of this line indicates it was there from the beginning,
and was once caught as typo. Instead of replacing the line with the
correct line, the correct line was added...

MAC_BSSID_DW1_BSS_ID_MASK must be set to the mask 3, to
enable 8 BSSID's. The MAC_BSSID_DW1_BSS_BCN_NUM is initialized
to 7 to enable the 8 beacons.

Ivo van Doorn 1ed7a17a8e rt2x00: Remove unneeded variable
The update_bssid is set only when BSS_CHANGED_BSSID is used,
but the check if that field is true is done later in the function
but also only when BSS_CHANGED_BSSID is set. This makes the
variable useless, as it can never result in a negative check.

Ivo van Doorn f1aa4c541e rt2x00: Write the BSSID to register when interface is added
For the Master mode case, we initialized the BSSID as the MAC
address, but never wrote it into the registers. This causes
Hardware crypto to break in Master mode when receiving frames
which require the BSSID to be filled in.

This is safe for STA mode since the BSSID will be initialized
to 00:00:00:00:00 at this point, but will be set to the correct
value later when the device associates.

Ivo van Doorn e4a0ab3487 rt2x00: Synchronize WCID initialization with legacy driver
Legacy rt2870 driver handles WCID differently then we expected,
the BSSID and Cipher value are 3 bit values, while the 4th bit
should be set elsewhere in an extended field.

After this, rt2800usb reports frames have been decrypted
successfully, indicating that the Hardware decryption now is
working correctly.

Ivo van Doorn e1b4d7b735 rt2x00: Enable fallback rates for rt61pci and rt73usb
Explicitly enable the usage of fallback rates for
the transmission of frames with rt61pci and rt73usb hardware.

Note that for txdone reporting, only rt61pci is capable of
reporting the fallback rates, for USB it is not possible
to determine the number of retries. However the device will
use the fallback rates, so it might still help in the performance.

Ivo van Doorn 55f9321a02 rt2x00: Fix RF3052 channel initialization
Update channel initialization for the RF3052 chipset.
According to the Ralink drivers, the rt3x array must be
used for this chipset, rather then the rt2x array.

Furthermore RF3052 supports the 5GHz band, extend
the rt3x array with the 5GHz channels, and use them
for the RF3052 chip.

Ivo van Doorn 4789666e13 rt2x00: Hardcode TX ack timeout and consume time
The calculated values for the ACK timeout and ACK
consume time are different then the values as
used by the Legacy drivers.

After testing from James Ledwith it appeared that
the calculated values caused a high amount of TX
failures, and the values from the Legacy drivers
were the most optimal to prevent TX failure due to
excessive retries.

The symptoms of this problem:
 - Rate control module always falls back to 1Mbs
 - Low throughput when bitrate was fixed

Possible side-effects (not confirmed but highly likely)
 - Problems with DHCP
 - Broken connections due to lack of probe response

This should fix at least:
Kernel bugzilla reports: [13362], [13009], [9273]
Fedora bugzilla reports: [443203]
but possible some additional bugs as well.

Ivo van Doorn daee6c092a rt2x00: Reorganize padding & L2 padding
The old function rt2x00queue_payload_align() handled
both adding and removing L2 padding and some basic
frame alignment. The entire function was being abused
because it had multiple functions and the header length
argument was somtimes used to align the header instead
of the payload.

Additionally there was a bug when inserting L2 padding
that only the payload was aligned but not the header. This
happens when the header wasn't aligned properly by mac80211,
but rt2x00lib only moves the payload.

A secondary problem was that when removing L2 padding during
TXdone or RX the skb wasn't resized to the proper size.

Split the function into seperate functions each handling
its task as it should.

The bss_info_changed() callback function no longer needs
to be atomic. Remove the scheduled work structure and
call into the driver directly.

Additionaly this makes the DRIVER_REQUIRE_SCHEDULED
flag redundant so it can be removed.

In the TX descriptor the Packet ID must not be 0,
so add 1 to the ID. The wireless CLI ID is not allowed
to be 0 either, but also must be initialized with the
wcid value rather then keyid.
