Fix a long standing bug with per device locking.
Each device has an associated spinlock for synchronizing access to
hardware and state information with the ISR. A single hardware card has
one or more devices.
Bug: Non ISR code correctly acquires and releases the per device lock.
ISR incorrectly always acquires and releases the lock of the first device
on the card.
The decoupled and list based nature of the ISR and deferred processing
interaction allowed this to work in normal operation. Exceptional events
like an application forcing hardware shutdown, reset, or reconfiguration
while active can trigger the bug.
Fixed ISR to acquire and release the per device lock.
One exception is manipulation of the GPIO card resource which is global
and effectively owned by the first device of the card. Non-ISR access to
GPIO resource is changed to use lock of first device on card.

Fix transmit bug that could drop send data if write() called close to
serial transmitter going idle after sending previous data. Bug is caused
by incorrect use of device information member tx_count.
Driver originally processed one data block (write call) at a time, waiting
for transmit idle before sending more. tx_count recorded how much data
was loaded in DMA buffers on write(), and was cleared on send completion.
tx_count use was overloaded to record accumulated data from put_char()
callback when transmitter was idle.
A bug was introduced when transmit code was reworked to allow multiple
blocks of data in the tx DMA buffers which keeps transmitter from going
idle between blocks. tx_count was set to size of last block loaded,
cleared when tx went idle, and monitored to know when to restart
transmitter without proper synchronization. tx_count could be cleared
when unsent data remained in DMA buffers and transmitter required
restarting, effectively dropping unsent data.
Solution:
1. tx_count now used only to track accumulated data from put_char
2. DMA buffer state tracked by direct inspection of descriptors
with spinlock synchronization
3. consolidate these tasks in tx_load() :
a. check for available buffer space
b. load buffers
c. restart DMA and or serial transmitter as needed
These steps were previously duplicated in multiple places,
sometimes incompletely.
4. fix use of tx_count as active transmit indicator,
instead using tx_active which is meant for that purpose

Add receive programmed IO mode to reduce receive latency
when using low data rates. The receive FIFO trigger
level of 128 bytes used in DMA mode creates excessive latency
when operating at low data rates. PIO mode is selected when user
application requests data in blocks of less than 128 bytes.

Add run time control for receive DMA buffer fill level to allow
application to control receive latency when using stream oriented serial
protocols that pass receive data to application only after a DMA buffer
fills. This was previously a compile time option, but run time control is
needed when application changes data rate (and latency requirements) or
uses different data rates on different ports.

Leave the transmitter in reset state after configuration so that transmit
signal is held at mark until transmitter is explicitly enabled by
application, otherwise transmitter sends idle pattern.

Improve the accuracy of TIOCOUTQ value as implemented in chars_in_buffer()
method by walking and counting tx DMA buffers, reading controller tx FIFO
level and accounting for controller tx shift register. The greatly
improves application control of transmit latency at lower data rates.

The bug is caused by switching to hung_up_tty_fops in do_tty_hangup. An
ioctl call can be waiting on BLK after testing for existence of the locked
ioctl handler in the normal tty fops, but before calling the locked ioctl
handler. If a hangup occurs at that point, the locked ioctl fop is NULL
and an oops occurs.
(akpm: we can remove my debugging code from do_ioctl() now, but it'll be OK to
do that for 2.6.23)

Flush the tty flip buffer when the line discipline input queue is flushed,
including the user call tcflush(TCIFLUSH/TCIOFLUSH). This prevents
unexpected stale data after a user application calls tcflush().

Fix tty_set_ldisc in tty_io.c so that tty->receive_room is only cleared if
actually changing line disciplines.
Without this fix a problem occurs when requesting the line discipline to
change to the same line discipline. In this case tty->receive_room is
cleared but ldisc->open() is not called to set tty->receive_room back to a
sane value. The result is that tty->receive_room is stuck at 0 preventing
the tty flip buffer from passing receive data to the line discipline.
For example: a switch from N_TTY to N_TTY followed by a select() call for
read input results in data never being received because tty->receive_room
is stuck at zero.
A switch from N_TTY to N_TTY followed by a read() call works because the
read() call itself sets tty->receive_room correctly (but select does not).
Previously (< 2.6.18) this was not a problem because the tty flip buffer
pushed data to the line discipline without regard for tty->receive room.

Remove 'active' field from tty buffer structure. This was added in 2.6.16
as part of a patch to make the new tty buffering SMP safe. This field is
unnecessary with the more intelligently written flush_to_ldisc that adds
receive_room handling.
Removing this field reverts to simpler logic where the tail buffer is
always the 'active' buffer, which should not be freed by flush_to_ldisc.
(active == buffer being filled with new data)
The result is simpler, smaller, and faster tty buffer code.

tty->receive_room to avoid losing data if line discipline is busy (such as
N_TTY operating at high speed on heavily loaded system) or does not accept

Remove TTY_DONT_FLIP tty flag. This flag was introduced in 2.1.X kernels
to prevent the N_TTY line discipline functions read_chan() and
n_tty_receive_buf() from running at the same time. 2.2.15 introduced
tty->read_lock to protect access to the N_TTY read buffer, which is the
only state requiring protection between these two functions.
The current TTY_DONT_FLIP implementation is broken for SMP, and is not
universally honored by drivers that send data directly to the line
discipline receive_buf function.
Because TTY_DONT_FLIP is not necessary, is broken in implementation, and is
not universally honored, it is removed.

Add custom HDLC idle pattern feature.
It allows the user to specify an arbitrary 8 or 16 bit repeating pattern on
the transmit data pin between HDLC frames.
In most cases the idle pattern is continuous ones or flags as supported by off
the shelf synchronous controllers and defined in the ISO3309 standard. Some
applications (radio/satellite modems, connections to legacy military hardware)
require non-standard patterns.

Fix memory leak caused by incorrect use of tty buffer facility. tty
buffers are allocated but never processed by call to tty_flip_buffer_push
so they accumulate on the full buffer list. Current code uses the buffers
as a temporary storage for data before passing it directly to the line
discipline.

Remove duplicate call to idr_remove() in ptmx_open.
Error during open can result in call to release_dev() followed by call to
idr_remove(). release_dev already calls idr_remove so the second call can
cause a stack dump in idr_remove()->sub_remove() flagging an attempt to
release an already released entry.
I reproduces this on a machine with a misconfigured X server (attempting to
restart multiple times rapidly) getting the same error as the 1st link
below.

The stack dump can occur on close (as well as open) as shown
in the 1st instance above, possible from something like:

process A - wake and call idr_remove on index 0
...
process B - close, release_dev, stack dump on idr_remove (index=0)
because entry already removed

Fix hole where tty structure can be released when reference count is non
zero. Existing code can sleep without tty_sem protection between deciding
to release the tty structure (setting local variables tty_closing and
otty_closing) and setting TTY_CLOSING to prevent further opens. An open
can occur during this interval causing release_dev() to free the tty
structure while it is still referenced.
This should fix bugzilla.kernel.org [Bug 6041] New: Unable to handle kernel
paging request
In Bug 6041, tty_open() oopes on accessing the tty structure it has
successfully claimed. Bug was on SMP machine with the same tty being
opened and closed by multiple processes, and DEBUG_PAGEALLOC enabled.

buffer space but does not immediately follow the allocation with more data
and a call to schedule receive tty processing. (example: hvc_console) This

Change locking in the new tty buffering facility from using tty->read_lock,
which is currently ignored by drivers and thus ineffective. New locking
uses a new tty buffering specific lock enforced centrally in the tty
buffering code.
Two drivers (esp and cyclades) are updated to use the tty buffering
functions instead of accessing tty buffering internals directly. This is
required for the new locking to work.
