PAGE_SIZE may have been defined by the C libary (musl-libc does that).
So avoid redefining it here unconditionally, instead only use our
definition if none has been provided by the libc.

The recent introduction of bi-endianness on arm/arm64 had the
odd effect of breaking virtio-pci support on these platforms, as the
device endian field defaults to being VIRTIO_ENDIAN_HOST, which
is the wrong thing to have on a bi-endian capable architecture.
The fix is to check for the endianness on the ioport path the
same way we do it for mmio, which implies passing the vcpu all
the way down. Patch is a bit ugly, but aligns MMIO and ioport nicely.
Tested on arm64 and x86.

In order to be able to find out about the endianness of a virtual
CPU, it is necessary to pass a pointer to the kvm_cpu structure
down to the MMIO accessors.
This patch just pushes such pointer as far as required for the
MMIO accessors to have a play with the vcpu.

Currently the only use of the periodic timer tick in kvmtool is to
handle reading from stdin. Though functional, this periodic tick can be
problematic on slow (eg FPGA) platforms and can cause low interactivity or
even stop the execution from progressing at all.
This patch removes the periodic tick in favour of a dedicated thread blocked
waiting for input from the console. In order to reflect the new behaviour,
the old 'kvm__arch_periodic_tick' function is renamed to 'kvm__arch_read_term'.
In making this change it is necessary to actively flush the emulated serial
console's output buffer after the guest writes to it, as otherwise flushing
only happens with terminal input. Similarly, it is no longer necessary to
flush the buffer when we process input.

Currently, only x86 has architecture command-line options (for setting
the BIOS video mode) however this is likely to become more common in the
future.
This patch adds some simple macros and a struct definition to allow
architectures to augment the command-line options with private
definitions. The BIOS video mode option (--vidmode) is also migrated to
the new framework.

When generating a device tree for a guest, it is useful to have a helper
for converting host addresses to guest addresses in order to populate
the device nodes correctly.
This patch adds such a helper, following a similar implementation to the
reverse translation function that already exists.

Running a guest with multiple banks of memory based above 0 causes the
guest_flat_to_host address conversion to fail, as it is assumed that
guest memory addresses are offset linearly from 0.
This patch changes the translation function so that the kvm_mem_bank
structures registered by kvm__register_mem are used to translate guest
addresses, rather than use an offset from the start of host memory.

When registering memory banks for a guest, it is useful to keep the
range information around for translating between guest and host address
spaces.
This patch adds a list of kvm_mem_bank structures to struct kvm, which
is updated when a new bank is registered.

This patch adds a "--firmware" command line option to "vm run". You can use
this to try to boot with SeaBIOS, for example:
./vm run --firmware=/usr/share/seabios/bios.bin \
--disk $HOME/images/debian_lenny_amd64_standard.qcow2
This doesn't boot yet for obvious reasons but at least people can now start to
play with external BIOS images easily.

Coalescing MMIOs is only required when we really want avoid having to exit
to userspace for every MMIO write, usually it's not the case so no reason
to coalesce every MMIO zone (I was also pretty sure it didn't always coalesce
zones)..

Add a --hugetlbfs commandline option to give a path to hugetlbfs-map guest
memory (down in kvm__arch_init()). For x86, guest memory is a normal
ANON mmap() if this option is not provided, otherwise a hugetlbfs mmap.
This maps directly from a hugetlbfs temp file rather than using something
like MADV_HUGEPAGES so that, if the user asks for hugepages, we definitely
are using hugepages. (This is particularly useful for architectures that
don't yet support KVM without hugepages, so we definitely need to use
them for the whole of guest RAM.)

This patch passes the initrd fd and commandline to load_flat_binary(), which may
be used to load both the kernel & an initrd (stashing or inserting the
commandline as appropriate) in the same way that load_bzimage() does. This is
especially useful when load_bzimage() is unused for a particular
architecture. :-)

Different systems will want different base kernel commandlines, e.g. non-x86
systems probably don't need noapic, i8042.* etc., so set the commandline up in
arch-specific code. Then, if the resulting commandline is empty, don't strcat a
space onto the front.

Currently, the SIGALRM handler calls device poll functions (for serial, virtio
console) directly. Which devices are present and which require polling is a
system-specific decision, so create a new function called from common code &
move the x86-specific poll calls into it.

Create a new arch-specific subdirectory to contain architecture-specific code
and includes.
The Makefile now adds various arch-specific objects based on detected
architecture. That aside, this patch should only contain code moves. These
include:
- x86-specific kvm_cpu setup, kernel loading, memory setup etc. now in
x86/kvm{-cpu}.c
- BIOS now lives in x86/bios/
- ioport setup
- KVM extensions are asserted in arch-specific kvm.c now, so each architecture
can manage its own dependencies.
- Various architecture-specific #defines are moved into $(ARCH)/include/kvm{-cpu}.h
such as struct kvm_cpu, KVM_NR_CPUS, KVM_32BIT_GAP_SIZE.

This patch changes the IPC method to use UNIX sockets instead of
signals.
This allows for more flexibility in moving data between processes, for example
it would allow avoid printing output in the terminal of the guest.

This patch seperates 'kvm pause' into 'kvm pause' and 'kvm resume',
'kvm pause' no longer acts as a switch between running and paused state but
instead just switches the instance into paused and reume is handled by
the new 'kvm resume'.

Use the newly introduced KVM_CAP_MAX_VCPUS to determine max VCPU count
the kernel side supports.
Using more VCPUs than the recommended limit will issue a warning, using more
than the absolute maximum will limit it down to the absolute maximum.
Note this patch contains a hack to define KVM_CAP_MAX_VCPUS and should be
removed when the kernel patch is merged with mainline.

When the singlestep is enabled the ioctl to sent out when the kvm_cpu
is initialized (kvm-cpu.c in the for loop that gets each vcpu built).
When the ioct goes out the CPU is sitting at the initialization vector
of 0xf000:0xfff0 on CPU #0 and 0x000000 on the other SMP CPUs. The
new host kernel code that handles setting the TF was changed in 2.6.32
and again at 2.6.38. 2.6.32 seems just flat broken, but 2.6.38 checks
that the linear address of the RIP matches what it was when the
KVM_GUESTDBG_SINGLESTEP flag was set. Because the kvm-tool doesn't
start the CPU at the initialization vector (0xfff0) (0x7c00 for the
MBR and where ever you guys map the linux kernel to) they don't match
and the host kernel won't set the trap flag.
Basically the debug and singlestep ioclts need to happen after the CPU
has been initialized. I moved kvm_cpu__enable_singlestep to happen in
kvm_cpu__reset_vcpu after the registers are set (EIP points to boot
address) and the TRAP flags get set and all is good with the world.
Singlestepping is disabled when the guest issues a CLI because the
Linux host doesn't support features in new Intel and AMD CPUs. We can
sort of "shadow" the interrupt mask and still get the CPU to trap out
at ever instruction even when the guest has disabled interrupts on the
CPU. I have to get the bios disk handler working completely first,
but that may be my next task so that we can trace all the CPU
instructions. My current hack is to just re-enable the trap flag
every time a VMEXIT occurs. I get enough instructions to get me by.
This patch fixes the problem by moving the kvm_cpu__enable_singlestep() into
kvm_cpu__start().
